Q: What is included in the Sportlink Physical?
A: The Sportlink exam includes the following:

Q: How much does the Physical cost?
A: The cost is $39.99 per student.
Q: How much does the EKG cost?
A: The EKG can be added to the Sportlink Physical for a total cost of $99.99
Q: How much does the Cardiac Life Screen cost?
A: The Cardiac Life Screen is $179.99. Physicals are discounted $20 for Cardiac Life Screen participants for a cost of $199.99.
Q: Does Sportlink accept insurance?
A: No.
Q: Does the school have to pay anything?
A: No, the service is free and convenient for the schools.
Q: Can the school receive any reimbursement?
A: No
Q: How many physicals can you do in an hour?
A: One team of Sportlink physicians examines approximately 40 students/hour. With two teams student flow increases to
approximately 80 students/hour. Signing students up ahead of time is
critical in determining student flow.
Q: How will the student athletes sign up?
A: Students schedule an appointment online at www.sportlinkUSA.com. In some cases we allow students to bring a check to the school. This requires a school administrator to collect the check, log into our web site, and schedule the appointment in the system. We encourage schools to promote online scheduling as it is more efficient and less burdensome on school staff.
Q: What kind of insurance does Sportlink have?
A: Sportlink requires all physicians to carry personal malpractice insurance.
Q: What kind of follow-up is included?
A: The school receives a report with each student's name and their pass or fail information. The school contact is
immediately notified of any failures.
Q: What do you need from the school?
A: The school is requirements are:
Distribute supplied inserts to the school's students
Distribute history forms to the school's students
Sizable location for exams (half of a gymnasium works, a full gym is ideal)
6 tables
20 chairs
Access to electricity, waste, and a copy machine (having a copy machine moved to the Sportlink location is ideal)
One school contact present at all times during the event
Minimum of 30 participants for the event (Sportlink reserves the right to cancel the event a week prior to the event date if the minimum of 30 participants has not been reached).
Q: Can our team or school doctor be involved?
A: Sportlink is flexible. We've used existing school doctors to help administer our physicals. Doctors can work as
a physician team member if they are trained through Sportlink. In certain circumstances, they can participate as
an additional station. Feel free to talk to us about how your doctor can be involved.
Q: Do you have any advertisements that we can use?
A: We do have a school newsletter insert. We are willing to place an ad in your school newspaper or link you to
our website. We can discuss your individual needs.
Q: How do you deal with failures?
A: The parents of the student receive a phone call on-site from the physician. The school contact is notified on-site. Students are given a failure notice to take home with recommended follow-up. However, it is the school's responsibility to make sure a licensed physician clears the student before participating in school sports.
Q: Is the information confidential?
A: Yes, other than the file at Sportlink, only the school will have a copy of the results.
Q: Does Sportlink sell any information to third party vendors?
A: Absolutely not. Information may be used for Sportlink's own research; however, no names will be used.
Q: Can our booster club help participate?
A: We welcome booster club help in building awareness of Sportlink's service within the school. However, we
provide our own physical examination team members.
Q: What do you do if students miss the exam?
A: We openly welcome students to participate in nearby Sportlink locations.
If you have missed an appointment or need to reschedule, please follow the instructions below. Reservations are valid for rescheduling at another Sportlink event location for 90 days post the date of the event.
To reschedule an appt, email reschedule@sportlinkusa.com the following information:

Event where student is currently registered
Event where student is rescheduling
Attach a copy of the receipt (if available)
You will be scheduled in the next available time at the event where you are rescheduling. A Sportlink representative will email you to confirm the rescheduled reservation.
Prior to 10 business days before an event, you may receive a refund for a cancellation. There are NO REFUNDS made within 10 business days of the start of an event.
